In the Health Information Department we strive to provide accurate, efficient, quality and timely service of all your health information needs. A few of the services we are here to provide for you include (but are not limited to): Disability forms processing, FMLA forms, special letter requests, general requests regarding your personal health records, copies of your chart or films, Impairment Rating Requests (Non-Work Comp ONLY), and Certification of medical records, etc.

Below are some details regarding some of our services.

 

PATIENT RECORD REQUESTS:

*We will print/prepare your records upon arrival. Please allow 10 – 15 minutes for processing time.

*We reserve the right to ask for a picture ID upon disclosure of any Health Information.

*We prepare copies of x-rays and MRIs on compact disk. These are a superior quality than a hardcopy reproduction.

*We will only provide copies of films that were actually taken at Bluegrass Orthopaedics.

*Any original films you may bring from another facility are the ultimate responsibility of the patient, Bluegrass Orthopaedics is not responsible for lost or damaged films.
