In the Health Information Department we
strive to provide accurate, efficient,
quality and timely service of all your
health information needs. A few of the
services we are here to provide for you
include (but are not limited to):
- Disability forms processing
- FMLA forms
- Special letter requests
- General requests regarding your personal health records
- Copies of your chart or films
- Impairment Rating Requests (Non-Workers' Comp only)
- Certification of medical records, etc.
