Requesting Your Medical Forms from Bluegrass Orthopaedics

We partner with MediCopy to securely process medical record requests and Disability/FMLA forms. MediCopy is fully HIPAA compliant and follows all state and federal regulations for releasing medical information.


Patients

Including an email address on your authorization helps speed delivery. Records sent by email or directly to another provider are complimentary. Most requests are completed within two business days after MediCopy receives them.

Disability & FMLA form requests are $30 for the first form and $15 for each additional form submitted at the same time.

Medical Records Requests - Frequently Asked Questions

How do I request a copy of my medical records?
Medical records requests are securely processed through our partner, MediCopy. Patients and third parties can submit requests online through the appropriate MediCopy form above.
How long does it take to receive my records?
Most medical record requests are completed within two business days after MediCopy receives the request. If you completed the “BGO Medical Records Authorization Form” and faxed it to Bluegrass Orthopaedics, it may take a few additional days for MediCopy to process your request.
Is there a cost to obtain my medical records?
Records sent by email or directly to another healthcare provider are complimentary. Fees may apply for certain requests in accordance with state and federal guidelines.
What if I’m unable to submit my request online?
If you are unable to complete the electronic request through MediCopy, a paper authorization form is available. Please note that electronic submissions are processed more quickly.
